What You'll Do

• Complete client assessments and develop individualized care plans.
• Conduct routine supervisory and quality assurance visits with clients.
• Provide ongoing clinical support and guidance to Care Professionals.
• Assist with Care Professional skills training, medication competency, and education.
• Update care plans as client needs change and communicate changes with clients, families, and office staff.
• Collaborate with physicians, case managers, hospitals, and other healthcare providers to coordinate care.
• Assist in resolving client care concerns and ensure high-quality service.
• Maintain accurate and timely documentation.
• Participate in the office on-call rotation as scheduled.

Qualifications

• Current North Dakota RN or LPN license in good standing.
• Strong assessment and communication skills.
• Ability to work independently while collaborating with a team.
• Excellent organizational and documentation skills.
• Home care, long-term care, or case management experience is preferred but not required.
• Valid driver's license, reliable transportation, and current auto insurance.
